Food Vendor Contract

Afro Soul Festival

Vendor must follow all rules and guidelines set forth by the Event Sponsor Five Star Women of Color:

  • Pay a nonrefundable fee of $400 early bird rate (until June 1st) for the right to be one of Food Vendors to participate at the Five Star Woman of Color "Afro Soul Festival".

  • All food vendors must provide the following (an original copy day of event):

    • Certified Food Service Manager Level I Card​

    • Food Service Facility Permit (Click here if you need a permit)

    • Baltimore County Temporary Food Truck Permit (Click here if you need a permit)

    • Baltimore County Food Truck License

    • Vehicle Registration

    • Food Truck Insurance (1,000,000 Food Liability Insurance)

  • Provide quality food product.

  • Staffed with clean, professional, and courteous Personal.

  • Provide a wide range of payment options for the festival patron(s).

  • Arrive at the agreed set up time instructed by the Event Vendor Coordinator. Set up time is between 9:00 am and 11:00 am.

  • Shut down and complete last food order at the agreed time instructed by the Event Vendor Coordinator. Clean up time is between 6:00 pm and 7:00 pm.

  • All products, displays, activities, signage, and personal conduct must be compatible with a family environment including the presence of children.

I hereby and affirm that all information supplied is true and accurate to the best of my knowledge, and belief.  Notice of any change of participation must be in writing via email within 10 days prior to the festival.

Liability Waiver and Indemnification

Vendor agrees to participate in Afro Soul Festival at their own risk. Vendor acknowledges and agrees that Five Star Women of Color, its officers, directors, employees, volunteers, and agents (collectively, the “Released Parties”) shall not be liable for any injury, loss, theft, damage, or other harm to person or property resulting from participation in the event, regardless of cause, including negligence.

 

Vendor hereby releases, waives, and discharges the Released Parties from any and all claims, demands, actions, causes of action, or liability whatsoever arising out of or related to participation in the event.

 

Vendor further agrees to indemnify, defend, and hold harmless the Released Parties from any claims, damages, costs, and expenses (including reasonable attorneys’ fees) arising out of or related to Vendor’s participation, including but not limited to claims of injury to persons or damage to property.

Food Vendors who are not approved by Baltimore County Department of Health will forfeit their deposit.
